Do you need to rent a car in St. Kitts?
Honestly, it depends on where you are staying and how long for. Here is the case for renting a car in St. Kitts, the case against it, and roughly where the money crosses over, so you can decide before you land instead of at the counter.
The short answer
Rent a car if you are staying four nights or more, if you are out on the south east peninsula, or if you are travelling with children and their gear. Skip it if you are here for a cruise day, a long weekend in Frigate Bay, or a resort week where the beach is the point.
When a car is worth it
You are staying outside Basseterre or Frigate Bay
The south east peninsula is spectacular and it is also a long way from anywhere that sells milk. Taxi fares there run US$30 to US$80 each way depending on the resort, so two trips can cost more than a day's rental.
You are here four nights or longer
Rates start around EC$122 a day. Somewhere around the third or fourth day, the rental costs less than the taxis it replaces, and every trip after that is effectively free.
You want to see the island on your own timing
Brimstone Hill, Romney Manor and Timothy Hill are all a comfortable morning's drive. With a car you stop where you want and stay as long as you like.
When a car is the wrong answer
If you are off a cruise ship, do not rent. You would spend a chunk of your day in port doing paperwork for the visitor's permit, and an island tour covers more ground in less time with somebody who knows where to park.
If you are on a resort week at Frigate Bay, you probably will not use it. Everything is walkable or a short fixed-fare taxi ride, and the car sits in the car park earning nothing.
If nobody in your party is comfortable driving on the left, in the dark, on roads with the occasional goat, that is a perfectly good reason to book a driver instead. Nobody is grading you.
What it actually costs, both ways
Renting

If you do rent, read this first
Every visitor who drives here needs a St. Kitts and Nevis visitor's driving permit. It is straightforward, but it is not optional, and it is the thing people are most often surprised by at the counter.
Driving is on the left. Roundabouts turn clockwise and you give way to the right. Fuel is sold in imperial gallons. Roads are good on the main routes and narrower once you are up in the hills.
Going to Nevis?
